  • Schedules A-F Reporting form templates (Microsoft Excel). Six spreadsheet templates in a single Excel workbook. Please download and save the spreadsheets to your local computer, complete them, and return them as email attachments to Joanne Miller (Joanne.Miller@ucop.edu). The complete statistics schedule for is provided below. Note that Schedules A – F are due September 10.
